These changes include increased work capacity, improved cardiovascular efficiency, and increased muscular strength, flexibility, power and endurance.Risks I recognize that exercise carries some risk to the musculoskeletal system (sprains, strains) and the cardiorespiratory system (dizziness, discomfort in breathing, heart attack). I hereby certify that I know of no medical problem (except those noted; attached) that would increase my risk of illness and injury as a result of participation in a regular exercise program. By signing this consent form I understand that I am personally responsible for my actions during my physical training, and that I waive the responsibility of Garden State Strength and Fitness LLC, Michael Kelly, it's contractors, and employees should any injury incur as a result of my negligence. Participation Agreement, Release, Assumption of Risk, and Waiver *I agree that I have had read the Participation Agreement in its entirety and that I have the capacity to sign this Assumption of Risk, Waiver, and Release from Liability. How Billing Works We offer both monthly training and non-recurring training sessions. Monthly Training Monthly training is billed at a flat monthly rate based on your reserved weekly training time. Your rate remains consistent each month, even if a month contains four or five calendar weeks. Billing runs automatically on the first of the month, and thirty (30) days' notice is required to cancel. Non-Recurring Sessions Single sessions or session packages are available for purchase. These sessions are non-refundable and expire six (6)months from the date of purchase. Cancellations All sessions require at least twenty-four (24) hours' notice to cancel or reschedule. Late cancellations and no-shows are forfeited. Sessions cancelled with proper notice may be eligible for rescheduling based on availability. If yourtrainer cancels within 24 hours, your session will be rescheduled, and an additional session will be credited.
